- Near the end of the movie, Commodus bends down to his sister lying down on the bed. Inches close of an incestuous kiss, he backs out.
- Male characters are frequently shown shirtless or bare-chested throughout the film.
- Scene where a man's bare butt is shown.
- There is a kissing scene last like 5 seconds or more.
Violence & Gore

- A man treats a graphic wound on his body, which we see up close and for several seconds.
- Although most of the violence in this movie is quite brief, it is still pretty graphic and highly realistic.
- The first arena battle includes a spiked ball on a chain getting smashed into someone's face. It is very brief, but there is a large blood spurt. This is by far the most violent scene in the movie.
- Brief moment showing a woman get chopped in half.
- Multiple scenes showing bloody violence, much of it in brief shots.
- There is a rather bloody battle near the beginning (as well as a couple other shorter ones), and a few bloody arena battles.
- Men are killed with blows and dull objects, and stabbed with swords, knives, and spears (often with brief blood).
- There are a few brief instances of dismemberment and couple of decapitation, we usually see a flash of red. An opponent is also cut in half by blades on a chariot's wheel during combat; violent, but brief.
- An attacking tiger is stabbed with a sword, no blood is shown.
- A man slices off a tattoo on his upper arm, with a bit of bleeding.

Sex & Nudity
- Commodus attempts to provoke Maximus by telling him that his wife, who was murdered, moaned like a whore when the soldiers repeatedly ravished her.
- The villain threatens to kill his sister's son unless she agrees to have a sexual relationship with him.
Violence & Gore
- In the scene of Commodus's death, Commodus charges at Maximus with a knife, only to be repeatedly punched and elbowed. There is some blood on his face, but nothing too extreme. He then comes at Maximus with the knife again, only for Maximus to grab it and stab it into Commodus's neck. You do not see the impact.
- A woman and her child are killed by crucifixion and burning, not seen. Horses briefly and bloodlessly run a child over (not heard) and we later see the blackened lower legs of two corpses.
Frightening & Intense Scenes
- Maximus dies of his wounds after the final battle. This is extremely emotional and depressing to watch.
- Maximus sees his family's dead bodies which could be intense to some viewers.
