- Mild action-style violence. Mario punches enemies (gommba's, koopas, boo's, and more) they disappear in a puff of smoke. Bob-ombs run up to Mario and then explode.
- When Mario's life runs out, he will fall over from exhaustion. However, if his life runs out while underwater, he can be seen clutching his own throat from lack of air, gurgling before his head ducks down lifelessly, with Xs marked in his eyes (showing that he has drowned). If his life runs out in the toxic gas maze, he will cough constantly before clutching his throat, struggling for air before dropping dead from asphyxiation, with his body twitching shortly thereafter.

- Due to how old this game is, the controls are a bit different to modern day 3D platforms which could be hard to get used to for modern players and/or frustrating to work with.
- Some of the death animations are quite disturbing. Additionally, the Game Over screen can be scary for some, showing Mario exhausted against a red background as creepy music plays, although he thankfully gets better shortly afterwards.
- Some of the music in the game can make some players sad.
- The piano that comes to life can make people jump. It comes completely out of nowhere and it's completely invincible, making it that much more effective.
- The giant eel could scare younger players.
- Some people might find the Bowser fights to be intense.

- At the ending Princess Peach kisses Mario on the nose. Other than that, nothing is sexually explicit in any way.
Violence & Gore
- Bowser is fought by grabbing his tail, swinging him in a circular motion and throwing him onto one of several large, spiked bombs which explode with impunity upon impact.
