Did You Know?

During the course of the investigation, they keep referring to the "tag out sheet" that should be on the radar controls. In actuality, the DANGER TAG is what should be on the radar controls. It informs operators to not energize the equipment. A tag out sheet is the sheet you fill out with what you are tagging, why, the position it will be tagged in (ie off, do not energize, etc) and the number of tags you need to ensure the equipment cannot be started. For example, in this case, the tag should not have been on the equipment, it should have been on the circuit breaker supplying the equipment. You want all power removed from the equipment. No power to the console, no power to the array. The tag was in the wrong location. If the breaker had been tagged, it would have been impossible for the person to start at the console because there would have been no power. Also, the tag MUST be affixed in a manner where it cannot "fall" off for just the reason seen here. In real life, the person who hung the tag and the person who "seconded it verifying it was correctly hung" would be responsible. See more »
