- There is only one sex scene. It is more shocking and disturbing than sexual.
- A woman masturbates; she's clothed, but it's clearly implied. A woman performs a sex act on another woman (viewers don't see body parts, but there's no mistaking what position they're in). A woman makes out with a bunch of men in a drugged haze. A man touches a woman's genitals while he's teaching her some dance steps; he also forces her to kiss him. Bare breasts are flashed.
- Lilly is seen rubbing a male dancer's genitalia through his form-fitting dancing tights.
- Two sex scenes, one scene of masturbation, and brief sexual dialogue.
- Thomas asks Nina to "touch herself" as a homework assignment to get in touch with her darker, sexual side.
- Nina is briefly seen wearing a bra.

- Some ballerinas go through painful physical therapy in one scene. We hear joints crack at one point
- Themes of suicide. The swan's suicide at the end of Swan Lake is discussed and is artfully depicted by a ballerina. A character theorizes that another character attempted to commit suicide unsuccessfully. A character supposedly dies of a self inflicted wound, but isn't concerned about dying.
- Perhaps the goriest part of the movie is when a creepy character picks up a sharp object and rams it repeatedly into her own face. We see it in full detail, with bloody wounds in her face and neck. This character appears shortly after in jump scares, but from a distance where we only see blood and not the wound detail.
- Contains strong violence and bloody injury detail.
- A character has awful hallucinations in which she sees herself tearing bloody strips of skin away from her fingers.
- A character looks under another's hospital covers and sees horrible stitched gashes, bruises and braces on her legs. The wounds are shown still fairly fresh and bloody. The character gasps, horrified.
- Several camera shots focus on Nina's battered feet. We watch as she attends to black, broken and bleeding toenails. (The overall effect depicts ballet as an art form that requires something like self-torture and mutilation in order to perform it.)

- Fairly frequent swearing, including "f--k," "s--t," "pr-ck," "d--k," "p---y," "ass," "oh my God," and "damn."
- 16 instances of "fuck", 2 each of "whore", "ass", and "shit", as well as 1 each of "pussy", "cock", "dick", and "prick". Also 3 uses of "Jesus" and 2 of "God".
Alcohol, Drugs & Smoking

- Two young women get drunk at a nightclub. They also pop pills. Another woman, embittered because she has been replaced, causes a scene after getting drunk.

- Lily smokes cigarettes, and shares one with Nina in one scene.
- Nina and Lily drink and take ecstasy in one scene. It shows the effects of the drug but in a very dark and unappealing light.
Frightening & Intense Scenes

- Many of the themes are stabbing, self-mutilation, disturbing hallucinations, injuries, and bloody images. This movie is NOT for the faint-hearted.
- There are about 3 jumpscares in the movie and they are very unexpected
- Suicide and attempted suicide are discussed throughout the movie. We see injured characters post suicide attempts. We see ballet's artful depiction of suicide.

- A character hallucinates about turning into a swan throughout the movie. These visions are increasingly disturbing and graphic.

Sex & Nudity
- During the club scene, Nina is talking to Lily about the issues she's having with Thomas and Lily tells her that "you just need to let him lick your pussy."
- Lots of masturbating and lesbian sexual encounters.
- Nina and Lily have sex. We see them undress each other before Lily performs Cunnilingus on her. Lots of moaning. While very explicit the scene is played to be disturbing rather than sexual.
- A woman masturbates and when she's done we see that her mother was seating next to her on a chair. She didn't realize it but some might find it disturbing.
- Nina (most likely imagines) Lily and Thomas having sex backstage. We see him on top of her on a table, thrusting forward. We see Lily's bra partially as she smiles at Nina. No nudity.
Violence & Gore
- One character stabs another with a mirror shard, and she spits out blood and dies. The survivor puts the body in a closet.
- Nina brutally slams a door on her mother's hand. And her hallucinations include watching herself metamorphosing into a swan: Her neck elongates, her eyes going completely red, goose bumps on her arms erupt into actual feathers, she pulls bloody black feathers from her back, the skin on her toes fusing together and becoming webbed. In one of these scenes, Nina begins to transform in her bedroom, and bangs her head on the bed, knocking herself unconscious.
