- After fighting your way into Mexico, a closeup can be seen of Irish briefly groping a male horse. Mostly done for comedics as Irish checks if the horse is "healthy."
- One stranger mission has blackmail photographs of a man engaged in a few sexual acts with a prostitute, but none show nudity. The photos are optional to view as well.
- If you look around in other peoples houses, sometimes you can see some paintings with blurry nudity. Not explicit
- A couple is shown laying in bed after sex.
- There are suggestive themes in the game such as the inclusion of prostitutes and their propositioning of the characters to perform sexual acts with them, although they can't be hired.
- Occasional graphic sexual dialogue.
- There are movie theaters that contain cartoons with sexual innuendos, something around the level of Family Guy or South Park. For example, one of the cartoons shows a man performing sexual motions on several objects, including a table and his hat.
- There is a scene featuring a lynched man hanging by a rope in a barn, completely naked. His buttock and pelvic area are all exposed (his genitals have been castrated and a black spot remains).
Violence & Gore

- This game is quite violent but it's a bit less than its 2018 successor, Red Dead Redemption 2
- A man is found wounded in the desert near his stagecoach, indicating he could have been shot.
- This game has a implemented euphoria engine, which involves npcs writhing in pain, or reacting to your gunshots in a very realistic fashion. Npc reactions to your gunshots are procedural, so the kills will always have a different response, npcs can stumble, crawl, writhe in agony, and hold wounds. This is optional however.
- Blood splatters up on The screen when you are shot.
- Gameplay involves frequent shootouts with minor blood splatter.
- When skinning dead animals, lots of blood flies onto the screen and sounds of flesh being cut are heard, although the actual skinning is offscreen. The skinned animals can be seen afterward.
- Infrequent depiction of cannibalism.
- Some cutscenes contain intense violence.

- One quick use of "motherfucker" it's when Irish says "Motherfeckin' Mary" but he replaces the U sound with an E instead since he's Irish
- Cocksucker, Cunt, and Slut are used.
- Irish says slut-dog once.
- Puta, Puto, Cabron, Greaser and other Spanish profanities are used.
- Whore can be frequently heard.
- The profanity in this one is way worse than the second one
- 31 uses of fuck, 4 uses of cunt
- Frequent use of strong language like "goddamn", "shit", etc. Infrequent, rare uses of "fuck"
- Language cannot be taken out of the game.
Alcohol, Drugs & Smoking

- Irish is portrayed as an alcoholic who's usually very drunk during the cutscenes he's depicted in.
- The player can order drinks at saloons and can get drunk.
- In the game you have an inventory, now you can buy these stuff from a doctor or general store owner, you can get stuff like medicine, snake oil and cocaine gum, now the game is set in 1911 (1914 after story completion) so medicine wasn't advanced
- Drinking and smoking throughout.
- A scene where a man injects himself with cocaine.
Frightening & Intense Scenes

- When out in the wild, there can be times where certain predators can sneak up behind the player and kill them instantly.
- Seth might be frightening for younger audiences
- Rated R for Strong Bloody Violence, Language, Some Sexuality/Nudity, Drug Material and Disturbing Images.
- A side-quest involving cannibalism.
- Some intense violence but nothing too graphic

Sex & Nudity
- The male and female nudity can be seen in the opening cutscene of the mission "The Gates of El Presidio." It can be skipped; however it is very sudden.
Violence & Gore
- At the game's conclusion, the main character is graphically shot more than two dozen times all across his body. He is seen bleeding out, and can be heard gargling blood as he falls to the ground and the main character says the f word and he dies
- One character commits suicide by falling from a cliff.
- In the penultimate mission of the game, the protagonist, John Marston is shot to death by around a dozen men, very graphic
- after the game in the epiolouge jack tracks down edgar ross and guns him down getting revenge for his father
Profanity
- Damn and Hell are heard along with Bastard and Son of a Bitch.
Frightening & Intense Scenes
- John's death is very depressing.
- The assassination of the main character at the game's conclusion is shockingly, disturbingly violent. His family returns to find his bullet riddled body bleeding on the ground.
