We have two stories here. House, of course, is addicted to pain killers because of his throbbing leg. Early on, the dispensary runs out of his pills (which he gets in a questionable way). He is furious. Cuddy, makes him a deal to get him to go off the drugs for a time. He becomes more tyrannical than ever.
The case this time is wealthy kid who has been in an accident (his girlfriend was driving really fast) and has intense bleeding issues with lots of other complications. At one point he loses the sight in one eye. The problem is that no matter how many positives, he regresses. His father becomes furious at House because he sees his random actions and his antagonism as really inappropriate and dangerous. House, of course, has no interest in his feelings. His legs hurt and he can't get his Vicodin. This was a really intense episode.

Misdiagnosis is the name of the game with this show. A world famous trumpet player has ALS, or so it would seem. He collapses from his wheelchair while attempting to help out some musicians. Because he feels he can't play anymore and can't walk, he signs a DNR order. When he needs to be intubated, House ignores the order and helps him. Of course, this brings in the lawyers and he is hit with a lawsuit and a restraining order. When the tube is removed, he recovers his breathing, as his big shot lawyer from LA looks on. Now that he has survived, things take a different turn, and the real medicine starts. There is some serious adversarial stuff going on with Foreman who would like House to be nicer to him. There is a great scene between the two of them. Very good episode.
