In the free-falling section of World 5-2 where the player must grab the POW Block in order to advance while avoiding the spikes just before the boss fight against a Red Birdo, the underground theme is playing instead of continuing the ground theme. This also happened in the Japanese-only Doki Doki Panic, in which it was originally intended to be underground but decided to be an above-ground area instead. Enhanced ports of this game released later changed it to have the ground theme continue playing while going to this section of the level.
The Japanese version that's titled "Super Mario USA" still has the cast of characters' names using their Western names in the ending credits.
During the character credits, Ostro and Birdo's name's are switched, leaving Ostro's picture labeled "Birdo" and Birdo's picture labeled "Ostro." The game's instruction manual also contains this error.
The game's box art featuring only Mario and the artwork of the cast and other depictions of Mario in the game's instruction manual depict Mario and Luigi with their overalls being red and green respectively to match their own caps with their shirts being blue like in previous games. However, their in-game sprites depict them with their overalls being blue and their shirts matching the colors of their caps. The artworks in the Nintendo Power issues have accurate colors to their own shirts and overalls to match with their in-game appearances.
The boss of World 5 Clawgrip's name is misspelled as "Clawglip" in the ending credits. This is because the Japanese "R" sound is equivalent to the "L" sound in other languages.
