When Malloy is performing CPR, he is pumping the man's stomach instead of the heart.
The paramedics that arrive at the fire do not do their job competently. They rush to help victims of a fire, bring along a bag of equipment, but all they do for the most seriously affected victim is pick him up by his supposedly broken leg to get him onto a cart. They never open their bag of equipment. They are told the old man had swallowed a lot of smoke, yet they never administer oxygen or aid his breathing in any way. His broken leg is thrown around repeatedly as they have him situated on the cart, but they never splint the leg. They do not even attend to the other injured party. And lastly, one of the paramedics doesn't seem to know how to load the cart into the ambulance, lifting the cart by using the device for pulling the cart while walking. UPDATE in the above not being a factual error goof: while the ambulance attendants may not have performed their duty perfectly, they are nothing more than ambulance attendants, not paramedics. The Wedworth-Townsend Pilot Paramedic Act did not go into effect until 1971, so at this time there were no paramedics.
After Malloy kicks open what was the locked door, there is no visible damage either to the door or the door jamb.
The Actual LAPD response for "Code 2" is urgent with lights but no siren although it is cited (by Malloy) as "no lights or siren" in the series.
