(It's completely hidden, probably to avoid disturbing the viewers with the sight of a needle entering someone's arm.) Actual inoculation would have used smallpox pus forced into the skin with a small barbed disk. A scar formed at the inoculation site, usually the shoulder. You sometimes see these scars on older people.
