In the scene when the supposed killer (who can't been seen on screen) is walking around the reception area on the 3rd floor and then supposedly walks over to the outside of Amanda's office where it shows through the CLOSED double doors, Dr Sloane and Amanda inside talking about the different victims and the timing of the murders but then in the next scene Steve walks straight in to the same office through the right hand door which is now already open, not having opened it himself.
During the murder committed in the intro sequence, the respirator is turned off but no nurse comes in to check. When turned off it should have sent a notice to the nurses station.
While dealing with the young lady covered in gold paint who was participating in a James Bond Goldfinger fantasy with her James Bond role playing boyfriend, Dr. Sloan correctly states that suffocating because the body can't breath through the paint is a movie created myth. However, the young lady is having trouble breathing and he states "She's suffocating." Dr. Sloan then explains that it's due to the fact that the paint doesn't allow the chest or stomach to expand when it dries. The amount of paint on the patient is covered in is very light, and would never prevent one from inhaling even of it were heavily applied.
