When David drives the van out of the prison, it has a white license plate. A few moments later, the van is seen on the freeway with a blue license plate. When next seen, the plate has changed back to white again.
In one scene, the Hulk, who is played by Lou Ferrigno, is seen sitting on a rock with a little white dog licking his wounded hand. As the camera filming him starts to fade out to the next scene, Lou Ferrigno can be seen breaking character and laughing hysterically, likely due to the dog cleaning his hand.
As Phil Giles, played by actor John Steadman is driving the two correctional officers in his truck telling them what happened, he finished his line with "till I could pick my spot", which was actress' Gloria Gifford's cue for her lines. While she remains silent, actor John Steadman can be seen elbowing her arm to remind her that she had the next line. Gloria Gifford stated in an interview years later that she had been nervous riding in an old truck with faulty brakes, causing her worry, and not thinking about, or remembering her cue for the next line. In close-ups of the Hulk's wounded hand, you can see the green paint blistering on the back.
On the second hulk out you can hear the trousers ripping when they had already ripped from the first hulk out.
