- Holed up in a ghost town with a deluded woman, Lazarus and the woman must defend themselves against five men who want Lazarus dead.
- It is May 1866, and Lazarus meets Elias on the trail. Other men appear and start shooting. Elias goes down. Lazarus takes one in the leg but kills two of three attackers. One gets away. Lazarus swears something to man who dies.
Lazarus takes the body to a nearly-abandoned town. Sees a Lazarus grave marker. Passes out. Falls off horse. Wakes up in bed. No one else about. Gets dressed. Looks around the buildings in the area. Meets Abby in the Cantina. She tells him she read his diary when he was unconscious.
Tobin Bell, the would-be killer who got away, meets Robert Foxworth in a bar. Hires him to kill Lazarus.
Lazarus gets up at night and visits stable where he finds Elias' horse and the artifact that was in his saddlebags. Abby previously said that the dead man was not found, so Lazarus wants to know how his horse got into her stable.
Foxworth travels with Bell and his two lackeys.
Lazarus and Abby make up. She is deluded and thinks the town is populated.
Foxworth rides into town. Lackey rides into town. Lazarus has a gun but no bullets. Lackey recognizes Lazarus and draws. Someone with a rifle kills him. Foxworth leaves. Abby tells Lazarus that another town resident did the shooting, but it was her.
Lazarus can see that five men surround him and Abby. He wants to run with Abby. She won't leave town.
Foxworth tells Bell that they need Lazarus alive to tell them where he's hidden the artifact.
Antagonists enter town. Lazarus shoots one with the rifle.
Lazarus gets one more after he enters a room and shoots a mirror.
Lazarus takes Abby to church. She resists but follows.
Foxworth dismisses the other men, saying: "You. You're not good at this. Leave now. Or die."
Darkness comes. Bell is back with Foxworth.
Foxworth returns to talk to Lazarus face to face.
Foxworth explains how he became an outlaw: "My wife, a person like that, can hold back the pain of the world. When they're gone it rushes in and devours you."
Bell gets hold of Abby while Lazarus and Foxworth are talking.
Lazarus follows. Finds them in a building with corpses of towns people. Throws lantern, starting fire. Lazarus is out of ammo. Bell says "You got nothing left" and is about to fire when Foxworth returns and kills Bell.
Looking at the dead bodies of the town folk, Lazarus says "Cholera."
Foxworth, feeling contrition for his crimes and the pain of the loss of his wife, wants to die in the flames. Lazarus pulls him out.
Abby explains that she had a son named Lazarus who died. It was his grave where Lazarus initially passed out.
Foxworth says to Abby: "If it's any help, I see the town the same way you do. Alive. Simply because you're in it."
Lazarus rides out. Foxworth and Abby stay in town.
