BJ says that a patient is "thinner than 3 2 beer". This refers to beer that is 3.2% alcohol by volume, which was the limit for alcohol near the end of Prohibition. Today, beer in this range would be considered "light" or "watered down" (thin).
The title is based on the Clement C. Moore's poem, A Visit From St. Nicholas (Twas The Night Before Christmas).
