This episode takes place in 1994, when a gay marriage had little legal status in Alaska. In 1996 same sex marriage would be banned in the state; in 1998 a state constitutional amendment would outlaw same sex marriage. It wouldn't be legalized until the Supreme Court ruling in 2014.
The condition that Joel diagnoses Maggie as suffering from is labyrinthitis, an inner ear disorder, which is now better understood than at the time this episode was produced. Two vestibular nerves in the inner ear send the brain information about spatial navigation and balance control. When either of these nerves becomes inflamed, it creates a condition known as labyrinthitis. Symptoms include dizziness, nausea, and loss of hearing.
