When Juliet leaves the convenience store with Lassiter, a strand of hair hangs down right next to her eye in medium shots, but in close-up it is tucked back by her ear. This switches back and forth two or three times as the shot changes.
When Shawn first spots the patient files, the name on Regina Kane's file is seen clearly, most of the files have numbered stickers on the left side with a few having them on the right and the lid for the box is to the right of the box. When Shawn moves in to search for Regina's file, he removes the lid to the left, no names are visible, and all of the numbers are on the right side of the files.
At the initial crime scene, Shawn when facing into the office has his arms on the door jamb. When seen from Lassiter's POV, his arms are by his side. This occurs over a couple of switches of POV.
When Gus reveals to Shawn his ghostly experience as a child & how he'd asked the ghost's name, Shawn explains that he (Shawn) had hidden a Walky-Talky in the wall after Gus' dad had replaced the insulation. When using a walky, one must hold down a button on the side in order to speak to the person holding the other device, therefore , Gus could not have asked the question.
The chewed toothpick in the car that was used to try to hit them was on the floor. However, when a chewed toothpick is seen in the crime photos of the murder, Shawn states that it was on the seat of the car.
The scene where Shawn and Gus are checking out the broken mirror and Shawn touches it, Gus (when the camera is showing his side/back) says something like "Enjoy your Hepatitis" but Gus' Mouth Never moves.
When Shawn and Gus discover the attic room of "Regina," there's a camera angle that includes a mirror that Shawn walks past that shows both his image and also the image of a crew member wearing jorts (jean shorts).
When Lassiter and Juliet walk out of a convenience store (18:05), the store has mail box that reads, "U.S. Mail Service." Curbside boxes in the U.S. only read U.S. Mail, and the agency is the U.S. Postal Service. Additionally the store has a sign that reads "Lottery Ticket Centre." Centre is the Canadian spelling for center. The show was shot in British Columbia but supposed to be Santa Barbara, CA.
When Shawn gives the file to Lassiter and O'Hara, and Gus tells them that Shawn is sensing that "She's going to kill again" correctly keeping consistent with the name on the file "Regina Kane," but O'Hara then asks "Who's he going to kill?" She should have asked "she" since as far as she knows this Regina is a woman, since the police are not privy to the whole Dissociative Identity Disorder of Robert.
When saying goodbye to the boys, Dagmar says "Spaß haben" which translates as "To have fun". The correct German phrase would be "Viel Spaß".
(at around 36 mins) As Shawn talks about the chewed toothpicks found he says that there was one found in the doctor's office just like on the "seat" in the car of the individual that tried to kill them. As he says this there is a flashback to the car that clearly shows the toothpick on the floor of the car not on the seat. A mistake that Shawn, a perfectionist on details, would not make.
