- TriviaCarla is the only person on Dr. Cox's "Always allowed" list. JD, Hooch, and Jordan are on the "Never Ever allowed" list although JD briefly removes his name. Everyone else, including a Dr. Rotinaj (Janitor spelled backwards, even though Janitor is on the list as well), are on the "Sometimes allowed" list.

Dr. Cox becomes the new Kelso and JD has to take the role of the new dr.Cox, as the doctor that always stands up against the chief of medicine.
After 8 years of seeing the characters develop this is the episode that makes you realize how things have really changed and if you are a huge fan of the series it will leave you with a lump in your throat.
The whole mentor dynamic is over, the tides have shifted and the relationship between Cox and Dorian is so different from the start of the series, the characters have changed so much, that I dare any big time fan not to get emotional to such a twist.
