After screaming at Tawaret, who screams at him and Steven, Marc finds himself back in Dr. Harrow's office in Putnam Mental Facility, screaming. Harrow tells Marc that he didn't shoot him, and Marc's mind is fluctuating between sense and nonsense, and Moon Knight is a reassuring fantasy that Marc has created in his mind. Marc insists that Harrow isn't really a doctor, and tells Marc to tell him how he came to be there. He then tells Marc that when Marc got there, he told Harrow that he was in a place like the office except in Egypt, and that he was with Tawaret, and asks if Marc thinks that's sense or nonsense. Harrow is encouraged when Marc says that it's nonsense, and admits that some people imagine a psych ward to protect their mind from a harmful reality. The doctor finds the fact that Marc is imaging Tawaret interesting, and her presence will allow them to understand the boy Marc was talking about. Harrow asks Marc to tell him about the little boy.
Marc picks up a crystal award and stands up, and Harrow signals for the orderlies. They inject Marc with a sedative, and Harrow insists that he's not Marc's enemy.
Back with Steven and Tawaret, and Tawaret wonders if the two men are twins. She tells Marc that he's dead and Steven figures that she's right. Tawaret welcomes them to the Egyptian underworld, and Steven realizes that Tawaret is there to guide them through their journey to the afterlife. She explains that the real appearance is too much for the human mind to comprehend, so they perceive it as something they can recognize. Marc figures that they're seeing the realm as a psych ward because they're insane, and opens the door to find Egyptian sand and he's on a barge. Steven joins Marc and figures that they're looking at the Underworld as it really appears.
Tawaret joins them and says that if their hearts are balanced, they will spend eternity in paradise, the Field of Reeds. She pulls out their hearts and weighs them on a scale, and if the hearts don't balance then they'll get thrown overboard and they'll be dragged down, and remain forever. Marc whispers to Steven that they should steal the barge, and Tawaret realizes that the hearts are incomplete and won't balance. She suggests that they go into the barge, which holds all of their memories, and show each other the truth, and balance their souls before they arrive at the Field. Otherwise their souls will be destroyed.
The two men go back through the doors and back into the psych ward. Steven looks through windows in doors, into the rooms beyond, and sees his memories. They hear a boy yelling, and go into a room filled with corpses seated at tables, and the memory belongs to Marc. The scale with their hearts is on a table, and Steven realizes that Marc killed all of the people there. Marc says that they were the worst of the worst and Khonshu wanted them punished.
Steven points out that the moving scales are slowing down, and they see a boy watching them. He goes over to the boy, who runs off and Steven chases after him. The boy goes into a room, and Steven goes after him and locks the door so Marc can't follow. Steven finds Wendy there, grilling, while Marc's younger brother is drawing a fish with one fin while his older brother Marc teases him. Their father Elias is putting together a playhouse, and Marc leads Randall off. Wendy tells Marc to keep an eye on his brother.
Steven follows the two boys through a nearby forest and into a cave, to play 'Tomb Buster'. It starts raining, and Marc tells Randall not to be a baby. They go inside and Steven steps on some bird bones. He goes after the boys, and realizes what is about to happen. Steven yells to the boys to come out, and not to go any further. When they don't respond, Steven goes in after them and yells that the rainwater is rising too fast.
Marc searches through the ward for an unlocked door, and comes to a door with a window inside, showing Wendy looking at him.
Steven hears the boy yelling them and tells them to follow his voice.
Marc finds a room of his family living room, Wendy sitting with the mourners at shiva. Steven is there, and Wendy says that she wants Randall back. A young Marc enters the room, and as Steven looks on in shock, Wendy tells Young Mac that he was supposed to keep Randall safe, and it's his fault for Randall drowning. Steven runs upstairs after Young Marc despite Marc's attempts to stop him, and finds another memory of Young Marc celebrating his birthday but Wendy refusing to join Young Marc and Elias. Elias tells Young Marc that Wendy isn't feeling well, and Young Marc blows out the candles on his cake.
Steven keeps running as Marc yells to him, and comes to a memory of Young Marc having another birthday. Wendy is there, drinking, and tells Young Marc that he was always jealous of Randall, and she should have known Young Marc would let Randall die. Young Marc runs to another room, and Steven tries to follow him and they both appear on a street. Steven wonders what Marc is hiding, and down the street a teenaged Marc is leaving home. Elias says that they'll fix it, and Teenage Marc says that his father is supposed to fix it, and asks why he hasn't. Steven breaks free of Marc and goes over, and hears his father tell Teenage Marc that he can't lose another son.
Marc tackles Steven, and they find themselves at a desert camp surrounded by bodies. Steven figures that Marc killed them, and Marc explains that he was kicked out of the Army and went to work for his old CO, Bushman. They were supposed to raid an Egyptian tomb, but Bushman changed the plan and Marc couldn't live with that. Abdullah is one of the corpses, and Marc says that he tried to get the archaeologist away but they didn't make it.
When Steven wonders what happened to Marc, Marc leads him into a nearby tomb where the mortally wounded adult Marc is crawling to an altar of Khonshu. He prepares to kill himself rather than die painfully, and Khonshu says that he's in search of a warrior. Khonshu wants Marc to become his Avatar and eliminate those who deserve it. The god can feel Marc's fractured mind, and says that Marc is a worthy candidate to serve him. Steven figures that Khonshu was taking advantage of the dying Marc, but Marc says that Khonshu let him continue being a killer. The adult Marc swears his allegiance to Khonshu. He heals and rises, and the raiments of Moon Knight appear on him.
The two men hear screaming and run outside onto to the boat. Tawaret is there, and tells them that unbalanced souls are being judged before their time, and it's bad. They figure that it's Harrow's doing, and ask Tawaret to contact Layla and reach out to Khonshu to heal their body. Marc figures that he doesn't want to go back to being Khonshu's Avatar, but they have no other choice. Tawaret agrees to help them, piloting the boat back through Osiris' gate which is the only way back. The goddess tells the two men to go back inside and balance the scales before they reach the gate,.
Inside the boat, Steven figures the last room is the key. Marc says they don't have to go there, and he'll tell Steven everything. He insists that it's not worth it, but Steven tells Marc that he's about to lose everything and if everyone else dies, including Layla, it's Marc's fault. Marc yells that he won't do it, and hits his head, and finds himself back in Harrow's office. Harrow calms Marc down, and says that he didn't have Marc injected with anything. The doctor tells Marc that he's proud of him, reliving his traumatic memories, and asks if Marc created Steven to hide from all the terrible things he thinks he's done in his life, or Steven created Marc to punish the world for what Wendy did to him. Harrow tells Marc that the only way he can know is to open up to Steven,
Young Marc is in his room, crying. Marc and Steven are there, and Steven doesn't remember the memory. Wendy pounds on the door and tells Marc to let her in. Steven takes over Marc's body for the first time, starts speaking in a British accent, and starts cleaning up the messy room. When Steven sees a poster of 'Tomb Buster' on the wall, he realizes that the hero's name is Steven Grant, and Marc made Steven up. Wendy breaks in and tells Young Marc that he's going to listen, takes a belt from a hook, and beats the boy.
Marc pushes Steven out into the ward, and says that Steven isn't meant to see it, and that's the point of Steven. Steven punches Marc and says that he's just something Marc made up. Marc tells Steven that he got to live a simple, happy life, and thought that Wendy loved him and was kind, and she's still alive. Steven has no idea what Marc is talking about, and tells Marc to let him out.
Marc is back in Harrow's office, with Steven in control of the body, and throws the water in his glass into the doctor's face. He apologizes and Harrow welcomes Steven. He asks if Marc had a chance to speak to Steven, and Steven says that Marc lied to him. The doctor explains Marc/Steven came there after Wendy died, and Steven insists that it's not true. Steven insists that Wendy is alive, and Harrow apologies for his "mistake" and dials up Wendy despite Steven's objections. Harrow gives the receiver to Steven, and Steven says that his mother is dead.
In the past, Elias is holding the shiva for Wendy and sees Adult Marc outside on the street, drinking from a flask. Elias motions Adult Marc to come in, but Adult Marc refuses and walks off, crying. Adult Marc finally falls to his knees, sobbing, and then Steven takes over Adult Marc's body and wonders where he is. He "calls" Wendy and imagines she's at the other end, alive, and says that he's lost again.
Marc appears and watches with Steven as their other self walk off, and Marc says that it's two months ago and that's when their lives started bleeding into each other. He couldn't face his mother's death, and Steven tells Marc that Wendy was wrong and none of it was Marc's fault.
The ground quakes and everything else stops moving. The Gates of Osiris appear on the street, and the two men run out onto the deck of the boat. Tawaret tells them that the scales never balanced and she can't stop the inevitable. The sand corpses climb out of the sand and board the boat, and Marc fights them but is overwhelmed. They start to pull him overboard into the sands, and Steven pulls the boat around, knocking them back. The sand corpses recover and continue attacking Marc, and Steven realizes that he's Marc as well and attacks.
One new corpse reaches over the side and grabs Marc, and Steven attacks the corpse and falls over the side. Steven is frozen into sand as the boat continues on, and the scale balance. Marc finds himself in the Field,
