- Post-birthday party, Fiona busts her lip open on her cast and Dr. O'Hara and Jackie stitch her up in the Peyton kitchen. Jackie notices Sarah Khouri's philandering behavior and brings up her concerns to Dr. O'Hara. Libby Sussman, the uber-patron of All Saints, is brought into the ER.—Showtime Publicity
- At home after a birthday party, Jackie and Kevin clean up a pile of silly string. O'Hara chases Fiona into the kitchen with a silly string shower. She sees Kevin not loving her being there and tries to compliment his macaroni and cheese. He essentially ignores her.
In the other room, O'Hara's sometimes girlfriend Sarah (Julia Ormond) dazzles fifth grader Grace with tales of her world travels. Fiona tries to get her sister to play and finally gets Grace to chase her, but the two end up wrapped up and Fiona's fake cast whacks her in the mouth. She starts shrieking and Jackie and O'Hara tend to her, over Kevin's objections that they go to the hospital.
Later in the kitchen, O'Hara prepares to stitch up Fiona as Grace looks on nervously. Sarah provides mock broadcast commentary to lighten the mood. Kevin doesn't look happy.
Later getting out of a car at the hospital Jackie checks that later they'll take out Fiona's tonsils on the porch. O'Hara makes dinner plans with Sarah for later and says good bye with a good smooch.
At work, Zoey nervously chews on a cross around her neck. Thor catches her praying and she admits that she made a deal with God that if she wasn't pregnant she'd make some changes in her life and work.
Lenny, the paramedic with a crush on her, comes over and Thor and Sam both observe as Zoey checks him out and compliments him on his newly tidied goatee. He thinks he has a shot.
Mrs. Akalitis supervises the gurney arrival of Libby Sussman, a very well dressed older woman in a tweed suit and pearls who makes a call on the way in. Thor explains to Sam that her name is on half the plaques in the building.
Mrs. Akalitis hovers as Jackie and O'Hara check her out. Mrs. Akalitis also checks to see if she's still donating money for the ER improvements. She seems fine after fainting at an auction.
Cooper strides in announcing: "I heard there was another celebrity in the ER." She shooes him off, calling him ridiculous and saying he looks like one of her grandson's action figures.
In the hall, Sarah finds Zoey, who is twitterpated at meeting the famous TV reporter and even more so by the idea that Dr. O'Hara has mentioned her to Sarah. She figures out Sarah's relationship with O'Hara slowly.
"It's like I can't see gayness, it's so annoying."
Sarah shows Zoey a brown recluse spider bite she got in Arizona two weeks ago. She wants Zoey to treat her without admitting her. Zoey gets all on the Q.T., telling her to meet up later in the bathroom.
Jackie draws blood from Mrs. Sussman as Mrs. Akalitis continues groveling for funding. Mrs. Sussman then snaps her fingers at Jackie for attention and asks Jackie to hand her her purse. Jackie explains that the snapping and fetching just isn't going to work.
Jackie walks down the hall and sees Sarah on her phone talking excitedly to someone. She asks Jackie to pass a message to O'Hara about dinner later.
Eddie calls Jackie asking if she's free and she says she can't talk. "Oh, we're doing this now?" he asks.
"I don't know what we're doing, right now I am working," she says, and hangs up on him.
Dr. Cooper walks by in a huff, still smarting from his Sussman brush-off.
Zoey gets the antibiotics for Sarah but is stumped by not being able to find a syringe. She calls Sam in for help under the pretense of chewing him out about the disorganization and gets what she needs. Sarah sneaks into the bathroom to meet her but Zoey is waylaid by Thor needing to freak out about a boy.
Jackie wheels a patient out with O'Hara and tells her what Sarah said. She mentions she was on the phone and adds, "If I didn't know any better, I would have sworn she was on the phone with you."
"Yeah, well, one in every port," O'Hara says ruefully.
Jackie asks Zoey to get a patient a cab and she says sure, but first ducks into the bathroom to tell Sarah she'll be right back and if anyone asks what she's doing in the bathroom to say she has to go to the bathroom.
Outside with the patient, Lenny comes by and asks Zoey out. She tells him about her new post-pregnancy scare philosophy on life about trying to "grab everything by the balls, but it's confusing because I don't really think I'm a ball-grabber." He asks her out again and she says "fine, maybe."
Zoey leaves Lennie to wait with the patient for the cab and he basks in his tepid victory.
Cooper tends to a man with an infected finger, fuming about having his "authority called into question" because of his looks. He asks Sam "attractive man to attractive man" what he should do about it and concludes he should say something.
Jackie busts Zoey going into the bathroom and gets updated on Sarah's spider bite. Jackie runs Zoey off and prepares to inject Sarah in the rear. Sarah sees the look on Jackie's face and confesses that she was in the states for a few weeks with someone else before she called O'Hara and just didn't want to deal with the questions. Jackie tells her she gets it, really she does, but O'Hara's her friend. "It's not like I don't love her," Sarah says.
Jackie checks on Mrs. Sussman, who takes issue with her not knocking. Jackie gives her the good news that she'll be discharged soon. Jackie's explaining it's not her job to call her driver when Cooper comes in and harangues her for questioning him in front of his colleagues.
He concludes with a "who the hell do you think you are?"
She reads him the riot act, explaining that just because he's on a poster doesn't mean he means anything.
"It's people like me who make doctors like you worthwhile at all, there are parts of this hospital that wouldn't exist without my support. I built them up like that and I can tear you down just as easily you silly little nit --"
And that's when Cooper grabs her boobs (his nervous tic). She demands he take his hands off her and he says he really wishes he could.
Mrs. Akalitis walks in to see the bizarre tableau and Sussman tells her she won't be getting a dime.
Later, Jackie and Cooper explain to Mrs. Akalitis that he's done this before. She tells him they're screwed without Mrs. Sussman's money.
Jackie and Cooper are trying to figure out how to fix things when Mrs. Akalitis calls for them. Mrs. Sussman is stroking out. Cooper doesn't want to take over chest compressions. "I don't think I should go anywhere near her chest," he says.
They give her CPR but she dies, and Mrs. Akalitis checks with Cooper and Jackie that it is their responsibility to carry out her dying wish and take $1 million from the children's wing and give it to the ER.
Jackie calls Eddie and tells him she needs some time to figure things out. He tells her to take time, but don't think he's waiting by the phone. She asks where is he. We see he's outside the hospital. He says he's at home.
Jackie leaves work and sees Sarah and O'Hara waiting for a cab then O'Hara getting upset and telling Sarah to just go without her.
Jackie comes home to Kevin and thanks him for staying up for her. He says they don't do that anymore.
She's on his lap and kissing him but he's not into it. She says she's ready to talk and he bitterly tells her he's not something she can cross off her list. He accuses her of not listening to him and says that they should have gone to urgent care earlier with Fiona.
He says she works too much and when she's home she's tuned out or focused on trying to fix things and not realizing what she's missing. He implies that Jackie is the one who makes their daughter Grace nervous.
She yells back, saying he couldn't possibly understand because she has no idea what it's like to be her. Kevin withdraws and leaves their bedroom.
Later, Jackie creeps down the stairs past Kevin and Fiona asleep in a chair.
She goes to a diner for pie. Alone.
