Sex & Nudity

  • Watson goes to a car he thinks is a Morse code and notices two people having sex although we see nothing except the car bouncing and maybe moaning, which is extremely mild.
  • The inn which Sherlock and John visit is owned by a gay couple who assume that Sherlock and John are also a gay couple. One of the innkeepers asks John about Sherlock's sleeping habits, insinuating that they sleep together.

Violence & Gore

  • A traumatised man unwittingly shoots a gun in the direction of his therapist, but only shatters the glass behind her.
  • A dog is shot and killed.
  • A rabbit is said to have been killed as part of an experiment.
  • Sherlock is seen covered in blood in a comedic scene, but the exact reason for it is never explained.

Profanity

  • Approximately 26 religious exclamations, 15 mild obscenities (the likes of "bloody" or "damn"), two derogatory terms (being the word "bastard"), and a few mild insults ("nutter", for example). On one occasion, the f-word is implied.

Alcohol, Drugs & Smoking

  • Sherlock is seen drinking liquor in an attempt to soothe his nerves in one scene.
  • A character smokes a cigarette in Sherlock's apartment. Sherlock, who is attempting to quit smoking, desperately inhales the smoke.

Frightening & Intense Scenes

  • This episode focuses on the nature of fear and is shot in a way which attempts to allow the viewer to experience the fear which the characters experience. It intends to be frightening and may be overly so for some viewers.
  • Especially the visions and the screaming of a traumatised character and a man wearing a leathery gas mask with big eye pieces tinted in dark red are what easily makes this the scariest Sherlock (2010) episode.

Violence & Gore

  • A character saw his father being killed as a child. In his own words, he was "torn apart". The episode includes several flashbacks of this memory, showing how the man was punched and strangled by another man wearing a gas mask before striking a rock and dying, but everything is lit and edited in a way that blurs most of the violence. A different version of this flashback has a big red-eyed hound as the assailant.
  • A character is close to comitting suicide, bringing a gun up to his open mouth and screaming in anguish, but he is eventually dissuaded from doing so.
  • A man steps on a mine and is killed offscreen due to the following explosion.

Frightening & Intense Scenes

  • A dog is killed with a gunshot (it's under very difficult circumstances).
