- The cat call whistle sound effect plays whenever the player achieves a certain combo
- There's some Crude Humor (Example, Peppino poops in a level). Some mild sexual innuendo.
- One of Peppino's taunts has him laying down without a shirt on with a rose in his mouth, it's meant to be humorous rather than sexual as it is a parody of the "draw me like one of your French girls" meme

- While there is no gore or blood, the violence is very moderate
- There's a lot of intense Cartoon Violence. Enemies get punched, stomped and shot. This is meant to be played for laughs. Some enemies bleed out pizza sauce (which is obviously a parody of blood).
- There is a rare chance that the enemys will scream in horror before you hurt them

- A few uses of "shit"
- A character named The Noise is showing a middle finger
- A few uses of "hell"
- The song that plays when Pizzaface catches you is called "Your fat ass slows you down"
- The "Whoop this" achievement shows Peppino giving the middle finger
- Brief uses of "Shit", "Hell", and "Ass". Characters never swear, it's just for some text.
- Some of the names in the credits are profane, including "pussybitch" and a name containing "fuck"
- When Peppino gets a P rank, he does the ombrella, an Italian gesture that means "fuck off".

- The "Penny pincher" achievement shows Peppino smoking a cigar
- The level "Fast Food Saloon" has beer bottles scattered throughout the level.

- Pizza Time is very intense.
- If you wait for about 40 seconds in the title screen without any movement Peppino will jumpscare you

- During the final boss battle, Pizzahead pulls out a pinup picture of himself in a bikini, it's meant to be played for laughs.
Violence & Gore
- In some levels you have a gun
- One of the toppin monsters in "Don't make a sound" has blood sauce leaking from its neck and face
- At the climax of the final battle, Peppino performs a piledriver on Pizzahead right into the tower.
- At the noise update after defeating the doise when you get back it shows doise's dead corpse and it has blood.
- In The noise update at the ending of the final boss the noise holds a huge noise bomb (or a nuke) and throws it at pizzahead.
Profanity
- At the fourth floor of the tower, there is a level called "Oh shit!"
- Some of the names in the credits consist of swears

- The Oktoberfest jumpscare has a character holding a beer
- During pizza time in the level "fast food saloon", Gustavo is drunk
- During the final battle, Pizzahead holds a pipe in his idle animation, he doesn't use it though.

- The gimmick of the level ''Don't Make A Sound'' is Jumpscares
- The Fake Peppino boss fight is very frightening, the chase at the end of it is incredibly stressful
- If you hurt Peppino a certain number of times, creepy things will appear on the tv hud
- Some bosses can be very scary (I.E Pepperman and Fake Peppenio).
- The song in the final boss's second phase may unsettle younger players.
