- Two years after her husband Jeffrey Miller died in a bomb explosion while serving in Afghanistan, Michelle Miller (Leah Gibson) now works as a field surgeon at Mercy Hospital, and lives with her son Bobby (Anthony Bolognese). Michelle was also a field surgeon in Afghanistan and her husband died on her operating table. Michelle fails to defuse the bomb in time, thus leading to Jeff's death and shrapnel scars all over Michelle's body. At Mercy, Michelle has the reputation of being solid under pressure and making quick decisions saving people's lives.
Meanwhile, FBI agents McBride (Caitlin Stryker) and Ellis (Sebastien Roberts) has arrested Ryan Quinn (Anthony Konechny), the brother of Sean Quinn (Jonathan Rhys Meyers) and the son of Patrick "Paddy" Quinn (Jon Voight), whose family is the member of the Irish mob called the Quinn Gang Brotherhood responsible for committing series of crimes in the United States. The FBI wants Ryan to testify against Sean and Paddy, but he refuses and says that his family runs a clean operation. The FBI knows that Paddy has picked Ryan to run the enterprise, bypassing Sean, and they have also leaked that Ryan has cooperated with the agency.
While bringing Ryan to the custody, Sean and his henchmen - Johnno (Patrick Roccas), Danny (Mark Masterton), and Mick (Bradley Stryker) - ambush the convoy, kill the agents and injure McBride. Sean shoots Ryan twice in a close range. Ellis escapes and brings McBride and Ryan to the hospital. Michelle was supposed to leave with Bobby for a football game, but seeing the serious injuries coming in, Bobby encourages her to help stabilize the victims before leaving for the game. In surgery, Michelle finds that Ryan was shot with an exploding bullet. She extracts the bullet, which explodes inside the ER, but causes no further injuries.
Sean calls his father Paddy about Ryan, and they determine to go to the hospital to get Ryan out. Sean hides from Paddy that he was the one who shot Ryan. Paddy asks Sean to wait till he gets there. As they arrive at the hospital, Paddy goes in first and learns from Michelle that Ryan survived. As Paddy is waiting for the on-duty doctor Terrence (Bobby Stewart), Sean and his men decide to enter the hospital and kill the receptionist. This forces Paddy to take control of the situation and he orders to take the visitors and staffs hostage, while Michelle hides Bobby inside the gym locker. Paddy cuts Danny's ear for disobeying his orders. Terrence takes Sean and Paddy to the emergency ward where they had kept Ryan and McBride. Frank (Marc-Anthony Massiah) is a security guard inside the building who works with Michelle to disable the security cameras, so the mafia cannot track their movements.
Ellis manages to hide Ryan in the upper floor of the building. Not finding Ryan, Sean kills McBride by disconnecting the ventilator. Sean tells Paddy that McBride shot Ryan. The police arrive at the scene, but Sean threatens to kill every hostage if they attempt to storm the hospital.
Using her combat skills, Michelle kills Mick after a struggle. Michelle then uses the emergency staircases to search each floor and finds Ellis and Ryan. Ryan tells Michelle that Sean was the one who shot him as he is livid about being passed over to lead the family business. Ryan says that the only way to stop Sean is to get to Paddy. Ellis and Michelle move Ryan again, as Danny finds Mick's dead body and tells Sean. Sean tells Paddy that Ellis has to be killed else Ryan will not be safe.
Bobby is found and is taken hostage. Paddy calls Michelle to bring Ryan in exchange of her son's life. Sean puts a bomb vest on Terrence and relays the message to the police not to intervene before Sean activates the bomb, killing Terrence.
Paddy confronts Sean for jeopardizing the job. Sean reveals to his father that Ryan has tipped off their illegal business to the FBI, so that's the reason why Sean is going after Ryan. Paddy refuses to believe that Ryan is a rat and an informer. Michelle finds Danny making a bomb and, after the struggle, kills him. Michelle makes a deal with Sean for exchanging Bobby and Ryan. At the morgue, Michelle brings Ryan to Paddy and Sean, and has Bobby back in her arms. Ryan reveals to Paddy that Sean shot him point blank. Paddy confronts Sean about it, stating that he will never forgive him. When Sean confronts Paddy for his mother's death because of Paddy's negligence, Paddy attacks Sean, who shoots his father in the abdomen. Before losing consciousness, Paddy resents of failing Sean as a father.
The police SWAT storm the building, kill the remaining henchmen and rescue the hostages.
Ellis kills Johnno after the latter attempt to catch Bobby, who escapes into the tunnel, and Sean shoots Michelle, knocking her unconscious for a moment, before activating the time bomb and running away. Recovering, Michelle manages to diffuse the bomb in time before heading to the tunnel where Sean takes Bobby hostage. Bobby is able to stomp Sean's feet, allowing Michelle to kill him. Ryan and Paddy are taken to another hospital, and Ellis commends Michelle for her bravery before heading home.
