I quit following the show soon after I saw this episode. Sorry. I used to like "Royal Pains", but after the first four seasons it just lost its steam. The cases became more and more absurd, the character development went straight down the toilet, whatever realism there was was completely obliterated. "Vertigo" is a fine example of that. Seriously, it is just awful. There will be some spoilers.

OK, the patient was a young and passionate orchestra conductor, who loved his job, etc... Now, if you bother to look at my message board history, you will probably see my Soundcloud profile and you will see that, although I still have a lot to learn, I know a thing or two about orchestral music - which is why I was appalled by what I saw here. First, sorry, but there has got to be a different way to show that a conductor loves his music. What we are presented here with is just mind-blowingly absurd. He was dancing... literally dancing... while conducting. Now, if you get the chance, ask a classical musician what they think about that. They will probably tell you that they want to be able to read the conductor's moves as easily as possible, which means that the conductor should not dance like an idiot while doing his job. Just look at Gustavo Dudamel - he is very emotional, but he is not dancing. When I see an orchestra conductor in a movie or a TV show dancing, I, in fact, don't see a conductor, I see a bad actor trying to play a conductor and failing miserably.

Second, mixing a new orchestral composition is sometimes a very hard thing to do, especially if you are using VST instruments as the guy here was. When you want to single out just one track, you don't do that by sliding down the faders of the other tracks on the sounboard, you just press the PFL button. Sliding down the faders ruins your mix. I know it looks good on screen, but that doesn't change the fact that showing it is about as idiotic as showing a renowned surgeon performing a complicated operation with a rusty chainsaw.

Third, does anybody really think a hospital's staff would allow an entire school orchestra to perform in one of the rooms? Because they wouldn't, you know that.

In other words, this episode is just insulting to anyone's intelligence.
