When a corpse decomposes, the body fats trap the odour, aerosolise and are spread through the surrounding air. The smell settles on a nearby person's clothes, skin and hair and it's difficult to remove even with heavy washing.
Ira is not competent in performing the duties of a rehab nurse. Letting an obviously wobbly patient fall during rehab (where they could injure themselves further) is not good practice.
