I've no clue why the female doctor is so giddy for a very violent person. I have never seen a movie that have absolutely no people to root for. They would've drugged Ronnie till the transport came.
For some reason this place has only one guard that let them go full violent till the last moment. A place with highly violent drug addicts will let them roam as they please. No one thinks to hid in the room they keep the drugs since it would be the most hardened area.
