- The few women in the game have their cleavage visible and there are some sexual comments in the game.
- A man is watching porn on TV. Nothing is seen, but moaning sounds can be heard coming from the TV.
- A few lines of Starkweather's dialogue implies that he is masturbating to Cash's executions.
Violence & Gore

- The player can not only blow an enemy's head off with weapons, but blow through their heads, leaving only the outline of their skull in very graphic detail.
- Some executions result in decapitations, sometimes quick, sometimes slow and painful.
- You often see blood, limbs and bodies of people who are implied to have been tortured and brutally executed in the game levels. One example is a building, in which has a room full of mutilated bodies hanging from hooks attached to the roof.
- The sound effects are pretty realistic, specially at the executions.
- Considered one of the most violent games.

- In this game, players will take on the role of a violent killer as he slaughters as many targets as possible. The executions that are performed are extremely violent. That said, the ways in which the player kills enemies are very brutal and disturbing.
- This game is not as bad as it's sequel Manhunt 2, but it's still the video game equivalent of a snuff movie. Each execution is very disturbing and violent. The amount of blood and gore that appears is honestly not excessive, but the styling of each kill more than makes up for that.
- Executions involving stabbing, cutting, beating, suffocating, mutilating, decapitating and shooting various people with guns, chainsaws, bats, knives, axes etc.. They are not excessively bloody or gory, but they are still very brutal. And what's worse is that you're rewarded for extra brutality.

- While there are uses of the f word, they are not as frequent.
- There is frequent usage of "Fuck", a few uses of "Pussy" and "Cocksucker". There is lots of milder language such as "Ass", "Bitch", "Hell" and "Damn".
Alcohol, Drugs & Smoking

- You can knock enemies out with a tranquilizer gun
- Some references in dialogue.
- In one scene, a tramp holds a bottle of alcohol in his hand but isn't shown drinking it. He also has a cigarette in his mouth.
- The Innocentz/Skinz drink alcohol and/or take pills, and smoke from a bong. Peyote is also mentioned.
Frightening & Intense Scenes

- It should be noted that this game has been extremely controversial for its graphic violence and has been banned in a number of countries, which shows how disturbing and violent this game is. The game has also been called a "serial killer training video" by some for obvious reasons.
- Although your victims are generally other criminals and no real innocents (no pun intended), that doesn't make it any less brutal and atrocious.
- If this game were a movie it would be Rated R for strong brutal violence including grisly images, disturbing content and for pervasive language.
- Rated R for Strong Disturbing Violence, Grisly Images, and Language Throughout.
- This game is rather intense, as stealth is the main way forward, and it builds up a lot of suspense. The atmosphere is also quite dark and menacing.
- The violence is very graphic and disturbing. Many players may feel sick or be disturbed by this game's blood and gore.
- There are some jump moments. Overall, they are not for the weak of heart.

Violence & Gore
- Towards the end, James Earl Cash kills Starkweather with a chainsaw. He slashes his stomach open, causing his guts and intestines to spill out (they look like a giant piece of beef), as Starkweather pleads for mercy, saying "I made you.", only to get ignored by Cash and his face destroyed with the chainsaw before Cash lodges the chainsaw onto Starkweather's eviscerated corpse. This is without a doubt the most violent scene of the entire game.
Frightening & Intense Scenes
- Each member of the protagonist's family is held hostage and executed in cold blood by a gunshot in the head. The scene is extremely graphic, and the characters' head get destroyed and deformed because of the shot, spilling a lot of blood after the shot is fired. The protagonist, watching it on a TV, gets angry and destroys the television.
- Piggsy is extremely disturbing and sinister.
