- A nurse is seen cowering in a corner with her panties pulled down.

- Several scenes early on that take place in the cafeteria that show some inmates suddenly going into a rage and throwing something, slamming on the unbreakable glass between he cafeteria and the kitchen, trying to attack each other, in each instance a guard puts them in a choke-hold to subdue them and drags them out of the cafeteria.
- A man tasers another man's eyesocket and eye (results are not shown).
- We see a thick trail of blood and gore after the decapitated man's body is pulled (offscreen) down a hall and into another room.
- A group of inmates take turns slamming heavy objects at the thick bulletproof glass that separates them from the kitchen. The window doesn't break, but because they are repeatedly using things like chairs, tables, and metal ladders to hammer at the glass, it comes out of the frame and the inmates rush the kitchen that way.
- We see the body of a decapitated man slumped against a wall, his severed head rests in his lap.
- A man gets a delivery of meat including chickens with their heads left on. Later the man accidentally drops part of the delivery on a tile floor, the meat inside bursts apart and meat, blood, and innards splatter everywhere. While cleaning it up, he sees some kind of bloody animal skull that's definitely not a chicken, it's much larger.
- A man is shown facedown on the floor, minus his head, there's a quick shot of the bloody stump and he has a very large pool of blood surrounding him.
- A man hurls a heavy telephone at another man's face, breaking his nose.
- A man is knocked unconscious when he's hit in the back of his head with a metal fire extinguisher.
- A man grabs and twists another man's broken nose, the man with the broken nose screams at the top of his lungs in pain.

- The violence is intense and almost non-stop with only brief respite. The entire movie is intense, and characters spend the last half of the movie badly frightened, some in total panic.
- The inmates are frightening and threatening. After the blackout, the suspense level is very, very high.

Violence & Gore
- A man kills a woman by skewering her in the stomach with a sharp metal pole.
- A man takes a knife used to dice meat, puts down one hand and starts dicing his own fingers, starting at the tips and working his way up to the wrist while giggling. Little blood.
- Two men drag another man out of his hiding place, he is shirtless and screaming and begging for help throughout. His entire head is wrapped in tinfoil with a hole for his eye. One man holds the shirtless man's arms, and the other drives a sharpened metal pole through his midsection. He is left to bleed out and dies soon after.
- A man pins another man down and drives his thumbs into the other man's eye-sockets to disable and kill him.
- A man grabs another man (both standing) by his hair and slams the back of his head repeatedly and violently against a wall. At some point the other man dies but the man keeps slamming his head into the wall until two other men pull him off.
- A man's nose is ripped off, with very bloody results. In his scenes after that attack, the lower half of his face is covered in blood and we see a gaping nostril hole.
- An unconscious, bound man is pulled towards a large stove with gas burners on the top. His friend, who is also bound, yells in panic at the unconscious man to "wake up". The two men lift him and lay him face down on the metal countertop and across all the burners. Just before they turn on the burners, he wakes up. His friend screams for the other men to stop, but they turn the burners on low, then on high so the flames come out of the burners, and he screams as he is cooked alive. He dies after clothing and hair catch fire. Later, there is a shot of the upper half of his body (brief), he is completely charred beyond recognition and some smoke is still rising from his blackened head.
- A man pours cooking oil over another man's shoulders and chest. He then uses a potato peeler to pare strips of skin from the other man's arms and body.
- A man puts his face very close to a dazed, frightened man's face.. He then slowly bites off the upper half of his own index finger. The camera shows his mouth in close-up, his teeth are bloody, he chews the finger. The whole scene is extremely graphic and accompanied by grisly sound effects.
