- Throughout the game, there are several instances of explicit sexual content, graphic nudity, extreme graphic violence, torture, disturbing images and explicit sexual violence. All of these are insanely disturbing, and it's not for the faint of heart.
- The entire game has an incredibly brutal difficulty (similar to that of the "Dark Souls" games) that makes it not for the easily frustrated. It's heavily reliant on trial-and-error gameplay, has aggressive enemies, and the main characters can either be driven insane, succumb to starvation, or be left permanently crippled.
- "Fear & Hunger" has a relentlessly bleak and cynical tone, what with its relentlessly tense atmosphere, a constantly looming sense of threat at every turn, a cast of psychologically broken characters, MANY horrific and HIGHLY disturbing encounters, numerous displays of SEVERE, aberrant and antisocial violent and sexual behavior, and more shown in explicit detail. As such, despite the game's gripping story and high quality, it must be approached with extreme caution.
- This is easily one of the most pervasively and explicitly graphic and disturbing video games ever made. Its content is extremely violent, extremely gory and very adult in every sense of the word, its setting is crushingly bleak, and its tone is overrun by a sensation of relentless darkness and experiment that evoke nothing but hopelessness and horror. It's a cold, brutal, harsh, and wholly unforgiving experience overall, so don't expect any happy endings here.

- If this game were adapted into a film it would easily be rated NC-17 for sequences of extreme graphic violence and gore, torture, gruesome disturbing images, extreme brutality, explicit rape, explicit sexual content, graphic nudity and some drug use.
