Sex & Nudity

  • The player can go inside the back of a building where images of scantily clad women in bikinis, and/or other states of undress are hanging on the wall. This is the main form of nudity in this game.

Violence & Gore

  • Zombies are killed by shooting them in the head, exploding their head.
  • Easily the goriest and most graphically violent game in the "Resident Evil" franchise. You can shoot zombies with a variety of weapons, and each shot leaves all sorts of gruesomely gory and detailed wounds on their bodies, not to mention that you can shoot off chunks of flesh from them, often leading to organs being spilled out and blood pouring everywhere.
  • When a zombie attacks the player an animation plays of them biting the their character.
  • This game is not only MUCH more violent than the original, but also a lot scarier. Much like "Resident Evil 7: Biohazard", the violence is exceptionally graphic and brutal, and the game has a dark and unpleasant atmosphere that makes it even scarier, relying not on the violence and gore, but on psychological horror, paranoia and a few jumpscares to make it genuinely scary, not helped by the game's extremely realistic and detailed graphics.
  • A man accidentally hits a woman with a truck.
  • Claire and Leon see a zombie attacking a cop.
  • The same truck driver causes a car to crash.
  • Marvin shoots a zombie to save the player character.
  • Dismembered and disembowelled corpses can be found around the map.

Profanity

  • Unlike the original, this game has your fair share of profanity that you'd expect to see in an M rated game. "Fuck" is uttered many times by several characters.
  • Other mild profanities "Shit", etc.
  • 1 use of the middle finger

Alcohol, Drugs & Smoking

  • Nothing noteworthy is used in this game.

Frightening & Intense Scenes

  • Much of the game's location takes place in a dark police station. The zombies lurk in the dark corners and sneak up on the player. Very creepy.
  • There's a character named Mr.X who can't be killed. He chases you after a certain point in the game. He usually gives up if you hide well enough, but too much noise (gunshots mainly) can make it easier for him to track you.
  • William Birkin is a man who gets mutated. Even worse, he is aware of what he's doing and has little control over it. Right after a cutscene, he can be heard screaming, "Help me!" and "Kill me!", and then his mutation gets worse and worse until he becomes a blob of flesh, teeth and tentacles with a giant eye in the middle. This is very disturbing and sad to hear and will definitely upset and/or scare players.
  • If the game was a movie, it would be Rated R for extended sequences of strong bloody horror violence and gore, disturbing/frightening images, and language.
  • Many monsters you come across are very horrific looking. The Lickers are particularly unsettling. The g-monsters encountered in the sewer section of the game are also very horrific due to there grotesquely mutated and deformed bodies.
  • Much gorier and more violent than "Outlast", but not as scary as that one. Nevertheless, this is without question one of the most violent video games ever released in recent years.

Sex & Nudity

Violence & Gore

  • The same woman turns out to be a zombie and bites the truck driver offscreen.
  • Elliot Edwards' bottom half is bitten off. It's not directly shown, but entrails are graphically shown as he dies.

Frightening & Intense Scenes

  • Leon and Ada enter Kendo's gun store to see that Kendo's daughter is infected. Ada states that they must kill the infected daughter but Kendo refuses and carries his daughter into another room. A gunshot is heard soon after, the body isn't seen but suicide is implied. For some it can be emotionally difficult to watch.
  • In Claire Redfield's story, she encounters a girl named Sherry, who's later kidnapped by corrupt police officer, Chief Brian Irons. Irons keeps Sherry captive in the orphanage, and there's a point where the player controls Sherry and must help her escape from the Chief. If you're captured by Irons while trying to escape from the orphanage, he will get you and the game is over: with the words 'You Are Trapped' written in front of a closed door. It's heavily implied that he is a pedophile. This level may be upsetting for some players, but it is a game meant for adults, so it's to be expected from a game in the horror genre.
