Warning: Spoilers
You have to pay close attention to Domina to fully understand what is going on, with multiple plot threads and lots of political scheming as the first season reaches it's halfway point.

"Plague" deals with a mystery illness - one that springs from an unlikely source - that befalls Gaius (amongst others) which threatens to cause a crisis which will will weaken Livia's position. Nothing like the imminent death of someone powerful to have the vultures circling.

Naturally, Livia has a few tricks up her sleeve. Kasia Smutniak is really good in the titular role. This episode, especially, shows her iron resolve. She isn't someone you want to mess with.

("Plague" also features the series' most graphic sex scene, a rape.)

In essence, an episode about fragility. One of virtues, the effort to give an universal definition of power, woman condition, her tenacity as result of profound vulnerability of position and, sure, remind of the rules of power. The old pain of Scribonia, the friendship between Livia and Agripina , the acceptance of the option for succesion made by his wife by Octavianus , the rape of Julia are few pieces in this sense. This universal message, transcending the frame of Ancient Rome can be the essential good point of series , against the obvious way of the so familiar succesful recipe and the good acting , against drops of soap opera and the inspiration from Rome series.
