You have to pay close attention to Domina to fully understand what is going on, with multiple plot threads and lots of political scheming as the first season reaches it's halfway point.
"Plague" deals with a mystery illness - one that springs from an unlikely source - that befalls Gaius (amongst others) which threatens to cause a crisis which will will weaken Livia's position. Nothing like the imminent death of someone powerful to have the vultures circling.
Naturally, Livia has a few tricks up her sleeve. Kasia Smutniak is really good in the titular role. This episode, especially, shows her iron resolve. She isn't someone you want to mess with.
("Plague" also features the series' most graphic sex scene, a rape.)
